I'd definately agree that you don't need a CMC to produce work like that, but can it be priced to cover the set up and time taken to make it? More to the point, will the customer pay it?

I go for the same price regardless of whether we fit the artwork or not! Good selling point to the customer if they don't want to mess around fitting artwork. But to be very honest, I don't get asked much for empty bespoke frames.
